Care Manager

Role

Care Managers help manage care for patients. They can also serve as individuals who provide healthcare to patients and supervise health care facilities. Functions of Care Managers in various primary care sites includes:

  • ​Create and maintain patient engagement in care within and across health settings

  • Address issues of health literacy, adherence and healthy living

  • Address social and economic barriers patients face in caring for their health

  • Keep information flowing between the patient and the rest of the healthcare team.

Training and Salary

Nursing, social work, or other health related training. However, many

of the jobs listed here can serve as care managers and roles/ requirements are site dependent.

$30,000-$70,000 (can have a wide range) dependent on training site
